Form widgets not painting in correct location

            Description

            Now that we're working back off the trunk we have started work on Forum support. The existing work seems to work but we have a handful of documents that do not render the form data in the correct location.

            As of yet no distinguishable difference or reason can be found between those that do render correctly and those that do not.

                Closing, our first take on widget annotation support is pretty much complete. Basic interaction, printing and form fill can now be done by the end users interactively or programmatically.
