For the sake of testing I decided to use this service to upload and download the file without making any modifications to it:
          http://www.pdfescape.com/
The file returned from the service was working properly in our ICEpdf viewer.
This looks like it have something to do with how the original data was saved.

The original PDF uses a AcroForm text fields to display the text. ICEpdf doesn't currently support AcroForm items officially. Normally the AcroForm field is backed by an annotation that will render the content but this isn't the case for provided PDF.
When I uploaded the file it is being touched by something on the pdfescape server (radpdf.com). And as a result extra xform objects are added to the document which is why the text is visible once retrieved from the server.

The PDF in quest is no rendering correctly in 5.2. The text widget annotations can now be edited in place and the contents visible.
