Errors rending iText 2.1 document

            Description

            The PDF in question was encoded using a fairly old version of iText, 2.1 I believe. There are two main issues.

            Each page has a nearly invisible FreeTextAnnotation on the top of each page. The appearance stream associated with this annotation is not correctly marked as a form object.

            The second issue is related to the a colour space parsing where an indirect object reference is specified where a dictionary should be.

            The issue are a bit strange as they are against the specification but they are also very easy to adjust for.
