Quite a few image changes have gone into the 4.2.2. release and the documents in question appear to load fine. JAI should also be on the class path.

This same customer has reported two new PDFs with the same problem. Perhaps the underlying issues are different but at first glance they seem similar. Attaching both PDFs.
