            Description

            the PDF in question has a mix of embedded and substituted fonts. It would appear the substituted font has an issue with assigning the correct glyph for a few characters like ' and ". Further analysis is needed.

              The root of the problem was a rough assert statement. The font in question parses fine and the correct ligatures are displayed.
