Render panel's header, content and footer as rows in a table to ensure that they are always resized together with a bounding box around all the panel parts.

Verified original issue ICEfaces 4 trunk r51921, Tomcat 8, MS Edge, IE 11, 10, 9, 8, FF 53, Chrome 60.
This fix has caused a regression in the showcase demo layout. The documentation menu is no longer right aligned in the demo header. See attached screen shot: align.png

Adjust panel's title style to use the entire width of the panel.

Tested with ICEfaces 4 trunk r51943, Tomcat 8. The showcase style adjustment was successful when viewed in Chrome 61, FF 53 and MS Edge as well as IE 8 through 11 when viewed on a windows 10 machine using Emulation in an MS Edge browser.

A style issue persists when viewing the showcase demo on IE 8 through 11 in a non emulated mode on the QA test machines, or in emulation from MS Edge on a windows 8 machine.

Let's consider this minor IE8-10 styling issue, that is specific to the showcase sample app a Known Issue.

It was discovered while verifying ICE-11330 that the commit made for this JIRA at r51910 causes the panel expansion and contraction animation to stutter. This can be demonstrated by comparing the Jenkins ICEfaces 4 trunk continuous build #2663 showcase ace:panel expansion and contraction motions against the same demo in the public showcase at http://icefaces-showcase.icesoft.org/showcase.jsf?grp=ace:panel

It would appear that the panel contents are no longer able to be resized by the animation, so the animated close goes from 95% to 0% open in one step. No good.

Use a DIV element (instead of TD) as the container that's being animated.

Verified panel animation resolved ICEfaces 4 trunk r51981, Tomcat 8, MS Edge, IE 11, 10, 9, 8 Chrome 61, FF 53.
Note styling issue still exists in showcase app (as shown in screen shot IE10.PNG) when using IE 8 through 11 (non-emulated).
