Failure to set initial graphic state clip

            Description

            This issue was reported in the forms. The submitted sample file is very unusual in that there is second content stream created with no content, well actually just a comment "%DSBlank". This lack of content is causing an null pointer when this content stream is attempted to be painted.

            There is a very simple fix which get the PDF in question rendering. I'll atach a patch file file and check in the fix to the trunk.
