This issue was related to PDF-197 and do to the work done in this bug this one gets fixed for free. Closing issue.

Definitely looks to be the same problem as
PDF-197. The potential fix I have forPDF-197works for the sample file they sent however they refer to a black image that is in a different file which we don't have. I'll see if the I can get he zero padding stream working and see if it fixes the banding issue for the patch.The black image is likely do to not have the correct JAI libraries on the class path.
