Description
The file in question has several problems from garbage bytes to an invalid xref table. The document appears to be created by an Adobe tool but has also ben touch by an older version of iText.

To simplify things I disabled the image proxy. On first load the xref error shows up but the page seems to render. If I leave the viewer open and close/open the file again the "reindexer" kicks in and the file loads without error. I don't have an explanation at this point for this behavior.
When the file is opened with Acrobat and then closed, Acrobat asks to save the document which usually means it has correct some issue with the file.
