Improve perceived performance by merging resources

            Description

            When <icecore:config mandatoryResource="all" /> is used, it would be nice if all the resources could be merged together and minified so that we would have a single javascript file and a single css file. This could be done only in production stage and the merged resources could be part of the icefaces jars. This would greatly reduce the number of http requests performed by browsers and improve performance in part due to limit of parallel open connections.
